Admissions Information & Application

We encourage parents who are interested in learning about The Sage School to begin exploring our program by attending one of our Open House events or by scheduling a private tour of the school.

Families who choose to move forward with the admission process, will be required to submit several key application items:

• The Application
• Intelligence test results (Stanford-Binet or Wechsler (WISC IV or WPPSI-III)) with a minimum full scale score of 120 I.Q.
• Copies of attendance records
• Prior school progress reports or report cards, evaluations sent confidentially from teachers

• Attend Admission Open House
• Complete your Admission Packet by January 15th*
  • Application Form (PDF) with $50 Application Fee
  • Intelligence Assessment by a licensed child psychologist
  • Student's School Records
    • Transcript Release Form (PDF)
• Teacher Evaluations (current year and previous year) (Teachers are to mail evaluation directly to The Sage School confidentially):
  Pre-Prime Applicants:
  • PreK-1 teacher evaluation form (PDF)
    Prime K and Prime 1 Applicants:
  • PreK-1 teacher evaluation form (PDF)
    Prime 2 Applicants:
  • Grades 2-5 teacher evaluation form (PDF)
    Junior Applicants (Grades 3-5):
  • Grades 2-5 teacher evaluation form (PDF)
    Middle School Applicants:
  • Grades 6-8 teacher evaluation form for Math (PDF)
  • Grades 6-8 teacher evaluation form for English (PDF)
• The Director of Admission will schedule the following visits with you.
  • Parent Interview
  • Student Visit**

We encourage parents to share their thoughts and questions with us throughout the admission process. We also ask families to adhere to the deadline of January 15th for submission of all paperwork so we may complete the admission process in a timely manner and process requests for financial assistance. As space allows, we accept applications after this deadline although financial assistance is typically not offered after the January 15th deadline.
